Does Polyhedron #12 have a pull-out insert?

Did Polyhedron #12 originally have a pull-out insert of any sort?
The Polyhedron Index doesn't offer sufficient detail for me to confirm/deny the existence of inserts. Example: My copies of issues 13-15 all have cardstock inserts still stapled to the center, but there's no mention of them on the Index web page.

I got curious and took a look.
I agree that 12 has no insert. Doesn't look like it ever did.
I have card inserts in 13 and 15 (Dawn Patrol and Star Frontiers aids, respectively). And then the modules start in 16.
I don't have a card insert in 14. What is it?

Thanks everybody for checking. That makes me feel somewhat confident that my issue #12 is complete.
The cardstock insert in #14 is a survey form. I'll see if I can scan it tonight, though relatives are descending on the house for a few days and it may need to wait until next week.

Please don't go to any trouble. Knowing it's a survey form is good enough for me.
