I know that MC1 & MC2 both came with binders and box sleeves and that MC3 Forgotten Realms came as an expansion with protective cover but....

... the MC4 Dragonlance Appendix came in a three ring binder and box sleeve also.

Is this true? According to the pic, which is of the outer box, it is.
If so, is the binder labeled Volume Three? Volume Four?

Does anyone have one of these versions?

I have one, only the binder, but I will have to dig it out (don't know where it is atm)
Hopefully someone can answer this.

Edit: http://en.wikipedia.org/wiki/Monster_Manual looks like the Dragonlance one wasn't labeled Volume 4

The binder is actually Volume Two and has a red dragon, death knight, and draconion on the cover.

I've got the items in question, so allow me to provide a bit of clarification.

TSR 2102 (MC1) Monstrous Compendium Vol. 1 (Binder #1)
TSR 2103 (MC2) Monstrous Compendium Vol. 2 (Protective Sleeve)
TSR 2104 (MC3) Monstrous Compendium Forgotten Realms Appendix I (Protective Sleeve)
TSR 2105 (MC4) Monstrous Compendium Dragonlance Appendix (Binder #2)

Thus, the second binder was NOT released with MC2, but with MC4.
