The earliest reference to the PO Box 756 address I can find is in The Strategic Review, Issue #3 (Autumn 1975) and  the first printing of Supplement II: Blackmoor (also 1975)
I suspect that this coincides around the time of the death of Don Kaye and restructuring "Tactical Studies Rules" into "TSR Inc."
The last reference to the PO Box 756 address I can find is in Dragon #213 (January 1995), concurrent with the 201 Sheridan Springs Road address. Starting with #214, only the Sheridan Springs Road address is listed in the magazine.

When I was in Lake Geneva talking with Ernie, I believe he said Luke now has PO Box 756.  :)

The transition to using that PO box seems to have started in the June-July 1975 timeframe. I see mail from mid-July with the PO box on it, but early June mail still shows 542 Sage Street.
