Does anyone have an actual product image of, AD&D Character Generation Dice Accessory (TSR 1155, possibly released in 1996 or 1997), and if so, wouldn't mind posting it here?  I've been through all of the mid-to-late 90s TSR catalogs but the only reference I can find to this product is either on a release timeline or a temporary pre-production artist rendition.

I don't think these exist. I have been looking for years and have never seen them.

Hopefully, someone will post a picture and prove me wrong.

I haven't seen this either. A bunch of those Dragon Dice expansions never got produced btw either.

I do have the Dragonlance Fifth Age Fate Deck and can confirm it exists.

The people at WotC (at least the ones answering my emails) don't know either (I thought it was worth a try given their TSR acquisition in '97).  Both Noble Knight and Amazon have product listings for them - so that makes me lean towards them being actually produced - but neither have pictures.  

Ladies and gentlemen, it looks like I may need to book a trip to Peru and pack a slightly overweighted bag of sand - just in case I come across these dice ;-).

But do you have all four versions? I understand that each one was packaged with a random one of four mini-adventures ("The Haunted Amulet", "The Dwarven Crown", "Death on the Deep" and "The Duntollik Run").

You know, just in case finding one single copy of the Fate Deck wasn't hard enough...  8O
