It was licensed when the Dungeons & Dragons Cartoon was aired for the first time in Italy

From the tacked-on look of the white dragon neck/head, I assume that the artist painted a red dragon, and then someone informed him, "No, we needed a FIVE-headed dragon. Didn't you get the memo? Just add another head and we'll call it a day..."
