I came across my old World of Greyhawk box and, of course, came here to figure out the printing. It's a 7th with "by Gary Gygax" under "Fantasy Game Setting." I did notice that there is a question regarding contents:
There are definitely two differing versions of the box itself. The more rare version has "by Gary Gygax" under "Fantasy Game Setting" on the front cover. Unfortunately, the contributor who told us about this is understandably unwilling to open the shrinkwrap on his copy, so the contents remain a mystery. The common version of the box is as depicted in the scan here, missing the "by Gary Gygax" line.
