I also have a catalog from the mail order hobby shop which lists B4's title as The Lost City Of Cynidecia. Again no pre-pub pics though. It also lists DQ1 as The Enchanted Wood. I am sure there are more items like these in the catalogs. If anyones interested I can research a little further.

I was looking through some dealer catalogues and came across this:
1. The L1 module's original title as "The Restenford Murders" (instead of "The Restenford Mystery")
2. (I think) UK1 module's original title and designation as S5 "The Enchanted Garden".
Interestingly, some years back I had seen a couple pieces of art that David De-Leuw had done for an adventure in 1981, and the title he wrote on both pieces of art was "The Enchanted Garden". One was an unfinished cover painting (wraparound for the cover folder?) and the other was the title page ink. I figured at the time that they were unpublished for "The Crystal Cave" but couldn't prove it without further information, though the subject matter seemed to match up with the module (if I'm remembering correctly).
EDIT: I found some other notes that I had written down, and it looked to me that for the cover painting De-Leuw had illustrated a combination of the encounters in the 'Garden Section' of the module at G10 with the Pixies at a fountain and G5 being stalked by a Satyr.
The interior ink (for the frontispiece/booklet cover art) was the encounter at G13 with a Centaur.
I think I understand now why David scrapped the two illustrations. He was illustrating Orlando & Juliana (the Romeo & Juliette couple) encountering the monsters where the two of them seemed nicely dressed and not wielding any weapons instead of art depicting an adventuring party tasked to locate and return the two of them.
3. Also "Dwellers of the Forbidden City" is listed as S4 and not I1
