I agree that the blank box bottom is probably the first print as well, I acquired one from an older game store owner that bought his copy the day it came out (he also got the brown boxset the day it came out as well).  It is just anecdotal evidence but evidence none the less

It probably did come in shrink though as Bill's copy (Stratochamp) is shrinked.  He got that at the gencon auction a few years ago for around $500 iirc

Does anyone recognize the multicolor pad of "Character Sheets" pictured in this overpriced OCE?  The distinctive spelling of "Expirence Points" at the bottom, may help identify them.

Not out of the realm of possibility that it's a home job. I recall BITD seeing some 'how-to' project for making your own tear-off pads in a school activity booklet. The type does not appear to be quite lined up flush, and any 'professional' job would have likely caught the spelling error.

Are the signatures real?

I expect that is exactly how he got them signed.  Monte Cooke recently signed a stack of books for me during the break in a charity game he was running at MobiCon.  One of the books in the stack had every major signature except his and original illustrations from several of the artists on the endpapers.  I think he enjoyed spending a few minutes looking that one over.  

If you have an opportunity to get into one of Monte's games, do so.  He is a great referee and typically the people willing to bid for the opportunity are experienced gamers.  It's pretty cool to game with an experienced group of strangers... as you don't know their inclinations.
